Core20PLUS5 is a national NHS-wide initiative to reduce health inequalities in a more collaborative way. The approach identifies ‘5’ focus clinical areas requiring accelerated improvement:
- Maternity
- Severe mental illness
- Chronic respiratory disease
- Early cancer diagnosis
- Hypertension case-finding
In addressing these areas, we are delighted to be working as part of the NHS’ innovative Complete Care Community initiative. The national programme is designed to support health systems utilise Primary Care Networks in tackling health inequalities. The programme centres on working with Primary Care Networks to identify the key health inequality challenges faced by local populations, in particular those associated with the wider determinants of health, that require collaborative working with local councils and other agencies.
As part of this Complete Care Community programme, ABC have initiated two projects led by the Equity team:
Sussex – Prostate cancer in men
Surrey – Early mental health diagnosis for asylum seekers
