- Have you had an appointment with a Smoking Cessation advisor?
- Do you have a recent carbon monoxide test reading?
- Complete your online referral.
What happens next
Patients will be contacted by a member of the Alliance for Better Care team to confirm eligibility for a clinical assessment. Patients will then receive a follow-up call and will be assessed by a member of our Clinical Team.
