BP at Home
We joined together with Surrey Health and Care Partnerships, as well as some local GP surgeries, to offer a blood pressure monitoring service that helped patients to monitor and manage their blood pressure remotely.
Each patient was referred to the programme by their GP practice. They were then contacted by a member of the ABC team, who explained the process and what they needed to do. Data was submitted either by the Inhealthcare App or via text. Results were then reviewed, helping to inform GPs’ ongoing care plans.
Discharge to Assess (D2A)
Discharge to Assess (D2A) was a service that focused on funding and supporting people to leave the hospital when it’s safe and appropriate to do so. Once patients have left the hospital, the goal is to continue their care and assessments, so that any support can be put in place for long-term care needs.
This service was needed when a patient is able to leave the hospital but isn’t well enough to return to wherever they were previously living. In this instance, patients are discharged through the Discharge to Assess pathway into a nursing care home at the Greathed Manor and Buxton Lodge, where they will receive additional support and any further assessments.
We supported care homes with the Virtual Ward Service and weekly visits at Greathed Manor and Buxton Lodge to assess patients and their needs.
Lipids Optimisation
In partnership with our practices, we offered a service for patients at risk of cardio vascular disease.
Eligible patients were offered a review with an ABC Clinical Pharmacist or GP with a view to implementing lifestyle and/or medication changes to reduce the risk of a cardiac event or stroke.
Our team worked with practices to identify the relevant patient cohort and will then manage all clinical activity thereafter.
