One referral. Personalised support.
We provide a single point of contact for serving personnel, reservists, veterans and Armed Forces families. Whether you need help accessing healthcare, support for your family, advice on housing or wellbeing, or simply don’t know where to start, our team is here to help.
Through one referral, we will work with you to understand your needs, coordinate support and connect you with the right services, so you don’t have to navigate multiple organisations alone.

Our programmes
Depending on your needs, support may be delivered through one or more of our programmes.
Single Point of Contact (OpCOMMUNITY)
Our dedicated team provides personalised support to serving personnel, reservists, veterans and their families, helping people navigate healthcare and access the right support at the right time.
We work closely with NHS services, Armed Forces charities and community organisations to ensure people receive coordinated, joined-up care.
Stronger Families, Clearer Pathways Project
Supporting families of wounded, injured and sick serving personnel and veterans.
Funded by the Armed Forces Covenant Fund Trust, our Stronger Families, Clearer Pathways project provides:
- Personalised family casework
- Healthcare and system navigation
- Accredited financial advice through West Sussex Citizens Advice
- GP practice training to improve recognition and support for Armed Forces families
By bringing together primary care, secondary care and community services, Stronger Families, Clearer Pathways helps reduce inequalities, improve continuity of care and ensure families receive coordinated support.
Thrive Together
While our Single Point of Contact supports individuals and families directly, Thrive Together works behind the scenes to strengthen support across Surrey and Sussex.
Through partnership working, training and service development we:
- Improve collaboration between NHS, local authorities, charities and community organisations
- Strengthen referral pathways
- Increase awareness of Armed Forces needs
- Help organisations become more Armed Forces aware
- Improve access to coordinated support across health, housing, welfare and employment
This means veterans and their families experience a more connected, easier-to-navigate support system.
