Dedicated specialists for every PCN
Our development model gives our Primary Care Networks (PCNs) the strategic leadership and specialist support they need to thrive and evolve. We work closely with PCN boards to shape services that meet local priorities and emerging population health needs, aligning with the national shift toward neighbourhood-based care. This reflects the NHS’s vision in its 10-Year Health Plan to bring care closer to communities, strengthen preventative approaches, and enable local multidisciplinary teams to deliver services outside hospital settings
Whether supporting Clinical Directors, strengthening neighbourhood working or building resilience across networks, we’re committed to helping PCNs deliver high-quality, locally responsive care.
