Discharge to Assess (D2A) is a service that focuses on funding and supporting people to leave the hospital when it’s safe and appropriate to do so. Once patients have left the hospital, the goal is to continue their care and assessments, so that any support can be put in place for long-term care needs.

This service is needed when a patient is able to leave the hospital but isn’t well enough to return to wherever they were previously living. In this instance, patients are discharged through the Discharge to Assess pathway into a nursing care home at the Greathed Manor and Buxton Lodge, where they will receive additional support and any further assessments.

ABC supports care homes with the Virtual Ward Service and weekly visits at Greathed Manor and Buxton Lodge to assess patients and their needs.
