Supporting patients to optimise their medication

In partnership with our practices, we are currently offering a service for patients at risk of cardio vascular disease.

Eligible patients will be offered a review with an ABC Clinical Pharmacist or GP with a view to implementing lifestyle and/or medication changes to reduce the risk of a cardiac event or stroke.

Our team works with practices to identify the relevant patient cohort and will then manage all clinical activity thereafter.
