Hospital at home
Virtual wards are a model of healthcare where patients receive hospital-level care in their own homes or usual place of residence, rather than being admitted to a hospital. We utilise technology like apps, wearables, and other medical devices to enable clinical staff to monitor patients’ recovery remotely.
Patients are cared for by a multidisciplinary team, including nurses, doctors, and other healthcare professionals, who can provide a range of tests and treatments.
Virtual wards aim to:
- Reduce hospital admissions and bed occupancy
- Improve patient experience and allow for more personalised care
- Reduce the risks associated with being in hospital
- Free up hospital beds for those who need them most
- Allow patients to recover in a familiar and comfortable environment
