Convenient service with remote support

We have joined together with Surrey Health and Care Partnerships, as well as some local GP surgeries, to offer a blood pressure monitoring service that helps patients to monitor and manage their blood pressure remotely.

Each patient is referred to the programme by their GP practice. They will then be contacted by a member of the ABC team, who will explain the process and what they need to do. Data can be submitted a number of ways including via the Inhealthcare App or via text. Results are then reviewed and inform GPs’ ongoing care plans.
